Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Do you have a passion for admission avoidance? To be successful in this role, you will need: Professional registration with the NMC or HCPC A mentoring qualification or working towards this Evidence of a range of clinical competencies appropriate for the role Experience in a variety of clinical settings Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct Demonstrable competency in a range of clinical skills Knowledge of current local policies informing community health and social care services A clear understanding of the role of the Nurse/Paramedic working in the community To beable to demonstrate the application of research based practice To beable to promote independence and choice in undertaking patient care IT literate To be able to demonstrate knowledge/understanding of boundaries of role Good numeracy and literacy skills Good communication and negotiation skills To be a car driver with a valid driving license for use in the UK and access to a vehicle for work purposes To be willing to undertake training to develop within the role To be able to reflect and learn from situations To be able to identify difficulties as challenges and works with others to identify solutions To be able to demonstrate a high level of understanding of self, able to identify personal limitations and shows openness to address them For a detailed job description and main responsibilities please see the attached supporting documents.
