JOB DESCRIPTION SUMMARY Responsible for the delivery of practice nursing care to the practice population. They will deliver evidence based care for patients presenting with long term conditions and provide preventative health care to the practice population. They will raise awareness among the population of the symptoms of long term conditions. They will be committed to identifying and reducing barriers to presentation to primary care services. As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating clinical decision making skills in the management of patients. They will work collaboratively within the practice team to meet the needs of patients, support the delivery of policy and procedure, and provide nurse leadership as required. Assess, plan, develop, implement and evaluate programmes which promote health and wellbeing and prevent adverse effects on health and well being Assess, plan, develop, implement and evaluate individual treatment plans for patient with a known long term condition. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate. Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including nondrug based treatment methods using a management plan, and in-line with national and local policies and practice needs Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC) Produce accurate and complete records of patient consultation consistent with legislation, policies and procedures Prioritise, organise and manage own workload in a manner that maintains and promotes quality Participate in the maintenance of quality governance systems and processes across the organisation Evaluate the patients response to health care provision and the effectiveness of care Support and participate in shared learning across the practice and the wider organisation Identify learning needs from patient complaints, clinical incidents and near misses utilising a structured framework Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of the statutory health procedure and local guidance Work within policies relating to domestic violence, vulnerable adults, substance and alcohol misuse; refer as appropriate Take responsibility for own development, learning and performance. Including participating in clinical supervision and acting as a positive role model Support the development of others in order to maximise potential Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Contribute and participate in the development of local guidelines, protocols and standards Ensure awareness of sources of patient support and guidance, and provide information in an acceptable format to all patients; recognising any difficulties where appropriate Ensure clear referral mechanisms are in place to meet the patient need Prioritise own workload and ensure effective time management strategies are embedded within the culture of the team Manage and assess risk within the areas of responsibility, ensuring adequate measures are taken to protect patients and staff Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken. Where appropriate, oversee the monitoring, stock control and documentation of controlled drugs usage according to legal requirements Apply infection control measures within the practice according to local and national guidelines Review and process information about patients using accurate read codes in order to ensure easy and accurate retrieval of information for monitoring and audit processes Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments Assess own learning needs and undertake training / learning as appropriate Provide an educational role to patients, families, carers and colleagues in an environment that facilitates learning Enable others to promote equality and diversity in a non-discriminatory culture The post holder will be required to undertake any other duties according to the needs of the Practice.
