We are seeking a dedicated and compassionate Community Social Worker specializing in Adult Services to join our dynamic Social Care team. Responsibilities
Conduct comprehensive assessments to identify clients' needs and strengths.
Develop, implement, and monitor individualized support plans in collaboration with clients and their families.
Provide direct support and guidance to clients in navigating social services and accessing available resources.
Facilitate referrals to appropriate services including health care, mental health support, and housing assistance.
Advocate on behalf of clients to ensure their rights and preferences are respected in all service areas.
Engage with community organizations and stakeholders to promote inclusive services and resources for adults in need.
Document client progress and maintain accurate records in compliance with regulatory standards.
Requirements
A relevant degree in Social Work or related field and valid social work qualification.
Demonstrated experience working with adults in a social care or community setting.
Strong understanding of social care legislation, policies, and best practices.
Excellent communication and interpersonal skills for effective client engagement.
Ability to handle sensitive situations with empathy, discretion, and tact.
Strong organizational skills and the ability to manage multiple cases effectively.
Commitment to continuous professional development and staying informed about current trends in social care.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.26
