Cheshire East Council (CEC) is looking for self-motivated Social Care Assessors to join our innovative and friendly Adult Social Care Teams. The positions are 37 hours per week working Monday to Friday.
About you
These posts will provide a wide range of learning and training opportunities and includes joint working across the Health and Social Care sector. It will provide you with an excellent opportunity to progress your career and will offer you a wide range of experiences within Adult Social Care services. The vacancies are based in the East of the Council and are part of supporting the Home First process. This involves working within the Adult Community Teams and supporting the Reablement service to support and assess individuals discharged from hospital or preventing hospital admissions.
The Adult Community Teams are part of the wider Care Community Partnerships with GP’s, Matrons, Mental Health services, Housing and voluntary sector services as they take a collaborative approach to supporting people within its footprint. The Community Teams provide support for any adults over the age of 18 years and includes adult safeguarding, those with a learning disability, autism, mental health concerns, physical disabilities and homelessness. This is an exciting opportunity for a Social Care Assessor to contribute to innovative ways of working and it is therefore essential that you have an appropriate Heath & Social care qualification and experience of adult social care and be able to work under pressure. The successful applicant will need to be able to adopt a flexible approach to their work and be a good team player.
The successful candidate will:
• Be passionate about providing a high-quality service to those in need of support from Adult Social Care
• Have an understanding of adult social care, mental capacity and safeguarding issues
• The candidates should be self-motivated with excellent communication skills and the ability to develop and work both autonomously within Adult Community Services and the wider multi-disciplinary arena.
• You will need to have good organisational and time management skills as this is key to effectively managing caseloads.
• The candidate should have an understanding of asset and strengths-based support and be able to remain calm in a variety of situations.
• It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.

The post holder will be based at Macclesfield Town Hall but there may be a necessity to travel to other bases including Delamere House in Crewe. The post will involve travel mostly across the East of the borough.
Cheshire East Council operates a ‘flexible and mobile’ working policy and hybrid working from home and office is available. Having a car is a necessity as you will be expected to undertake visits and attend training sessions.
