Proactively develop relevant treatment plans for patients requiring intervention including fluid resuscitation, oxygen therapy in accordance with patient group directives along with the ordering of appropriate laboratory and radiology investigations. Facilitation of discharged patients from critical care to support continuing recovery, coordinating all aspects of their care with the multi-disciplinary team. The management and coordination of patients discharged from critical care to wards with a tracheostomy to include weaning and decannulation. Responsible for the provision of a high quality professional nursing service across the Critical Care Outreach service, promoting best practice and implementing a review process that looks at quality and efficiency to meet the needs of patients and service. Supports the Critical Care Services in the development and implementation of policies, procedures and guidelines across the Trust service, ensuring that clinical governance is embedded in practice. Budget responsibility for the Critical Care Outreach Team for nursing monitoring variance and implementing contingency plans as appropriate. Ensuring service provision within the agreed financial resource. Support the Clinical Lead and General Manager in ensuring service objectives are met. Support the Modern Matron in developing nursing services to ensure agreed standards, targets and objectives are met. Responsible for the efficient management of processes e.g. critical incidents, complaints, monitoring of staff registration, training and development; and appraisal. Utilise information and IT systems to produce accurate and timely workforce and resource data and reports. Responsible for development, implementation and evaluation of projects to improve patient care and service development through the audit cycle, linking to the clinical governance framework and business planning process. Undertake direct workload requiring moderate to high physical effort according to patient dependency or staff need. Demonstrate through effective leadership the ability to provide professional and emotional support to individuals and groups, e.g. during difficult situations or unexpected events. Responsible for adherence to Health and Safety and Trust policy and Divisional objectives designed to protect healthcare staff and service users from clinical and non-clinical risk and known hazards. Contribute to and participate in the Trusts education strategy. Any other duties/tasks reasonably expected by the Trust Represent Nursing at Trust, Regional and National level.
