Summary


We are looking for an enthusiastic and experienced Band 7 Physiotherapist to work in the Brent Urgent Community Response Team.

Brent Urgent Community Response Team is seven days a weekly service, 365 days a year, working from 8:00 until 20:30 or 10:00 until 22:30. You will be expected to work weekends and bank holidays. However, it is our aim that you work approximately one weekend in four. Service aims to bring the best hospital care to a patient’s home and therefore avoid unnecessary hospital admissions for adults over the age of 18 years.

We aim to promote the very best patient care and provide the same care our patients would receive in secondary care. You will work as part of a multidisciplinary team alongside therapists, doctors, dieticians, and pharmacists to provide integrated and personalised care to the patients.

The successful candidate will have relevant experience working as a band 6 in acute and community settings. If you are that clinician with excellent communication skills, a team player with a positive attitude and committed to providing excellent care to patients, we are happy to have you on board.

The successful candidate will play a key role in staff development, contributing to service development and clinical skills training for registered and non-registered staff and supporting students on placement with the service.


• To undertake comprehensive Head to Toe assessments without supervision and to determine patient need/risk and plan, implement and evaluate their care with support from service consultant.
• In the absence of senior colleagues, take charge of the unit, day to day management of the team including taking handovers and helping to make treatment plans at the virtual ward round with the consultant.
• To work in partnership with multi-disciplinary members of the team, other professionals, and agencies, to meet the health and rehabilitation needs of patients within the Brent Urgent Community Response Team .
• To maintain standards of clinical care using an evidence-based approach within the service, incorporating principles and values of clinical governance
• Ensure effective communication is always maintained, whilst providing support, supervision, and clinical expertise to all members of the team
• Develop your skills into a more well-rounded holistic approach to patient care.
• Take on extended roles such as venepuncture, drug administration, advance assessment of the acutely ill patient and acute injury assessment.

Must hold UK HCPC registration and Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
