Role Summary We are seeking an experienced and compassionate Clinical Lead Nurse to lead the delivery of our proactive, community-based care service. This role is ideal for a nurse who thrives on mentoring others, driving clinical excellence, and making a meaningful impact in peoples lives outside of hospital settings. Key Responsibilities Clinical Oversight & Triage Lead clinical decision-making and triage care concerns raised by staff or clients Ensure timely interventions that prevent deterioration and hospital admissions Collaborate with GPs, district nurses, and other professionals to coordinate care Training & Mentorship Deliver structured training and ongoing clinical support to care staff Build confidence and competence across teams to deliver nurse-led care Develop clinical protocols and best practice guidelines Service Development Shape and refine the nurse-led model to meet evolving community needs Monitor outcomes and identify opportunities for improvement Promote proactive care planning and early intervention strategies Quality & Compliance Ensure care delivery aligns with CQC standards and clinical governance Lead audits, incident reviews, and quality improvement initiatives Uphold safeguarding, dignity, and person-centred care principles What Were Looking For Registered Nurse (Adult or Mental Health) with valid NMC registration Strong background in community nursing, clinical leadership, or care coordination Excellent communication, coaching, and problem-solving skills Passion for empowering care teams and improving client outcomes Ability to work independently and lead with confidence What We Offer A forward-thinking, supportive team culture Opportunities for professional development and leadership growth Flexible working arrangements The chance to shape a pioneering model of community care
