Be part of a forward-thinking team delivering virtual care across Sussex. Our Remote Monitoring Hub is a new and innovative service designed to support patients at home, reduce unnecessary hospital admissions, and provide compassionate care through digital solutions.
About the Role
As a Remote Monitoring Clinician, you’ll be at the heart of our virtual care service—triaging patient data, coordinating care, and ensuring timely, safe, and effective support for patients in their own homes.
Although the patient monitoring is remote, the hub will be based at Crawley Hospital, where the team will be responsible for the monitoring of patients across Sussex. The Remote Monitoring Hub Operational Service hours are 08:00hrs-20:00hrs, Monday to Sunday, 365 days of the year.

Your responsibilities will include:
• Triaging referrals and monitoring patient data
• Coordinating care with GPs, paramedics, and community teams
• Supporting patients and carers with empathy and professionalism
• Documenting assessments clearly and accurately
• Contributing to a positive, inclusive team culture
What We’re Looking For
We’re seeking a registered Nurse or Paramedic (NMC/HCPC) with:
• Strong clinical decision-making and triage skills
• Knowledge of frailty, heart failure, COPD, and diabetes
• Excellent communication and documentation skills
• A commitment to continuous learning and development
• Flexibility to work within a shift pattern
