Be part of a pioneering new service that’s transforming how we care for patients across Sussex. Our Remote Monitoring Hub is designed to support patients at home, prevent unnecessary hospital admissions, and deliver compassionate, timely care through innovative virtual solutions.

As a Senior Remote Monitoring Clinician, you’ll lead a dynamic team in managing patient data, triaging referrals, and coordinating care across community services. You’ll play a key role in shaping a positive, inclusive culture and ensuring patients receive the right care, at the right time.

Although the patient monitoring is remote, the hub will be based at Crawley Hospital, where the team will be responsible for the monitoring of patients across Sussex. The Remote Monitoring Hub Operational Service hours are 08:00hrs-20:00hrs, Monday to Sunday, 365 days of the year.

Your responsibilities will include:
• Leading shifts and supporting junior staff
• Triaging patient data and referrals with clinical precision
• Coordinating care with GPs, paramedics, and community teams
• Ensuring service quality and performance targets are met
• Promoting compassionate, patient-centred care

What We’re Looking For

We’re seeking a registered Nurse or Paramedic (NMC/HCPC) with:
• Strong triage and critical decision-making skills
• Experience in unscheduled care and team leadership
• Knowledge of frailty, heart failure, COPD, and diabetes
• Excellent communication, documentation, and IT skills
• A passion for innovation and continuous professional development
