Crynodeb
To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. To be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure. In order to work at this level the post holder must be a registered general nurse (currently registered the Nursing & Midwifery Council), have a recognised NP qualification (minimum of degree level or equivalent) and be an independent Nurse Prescriber. To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate. Provide expert professional advice to patients, carers and colleagues, with a holistic approach, and ensure the maintenance of clinical excellence. To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCT population. To provide education and training to other staff and students. Will act within their own boundaries of competence and agreed acceptable limits of practice to work as an autonomous practitioner Provide expert professional clinical advice and care to patients face to face, telephone consultations, and home visits ensuring clinical safety and excellence. The post holder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner including prescribing and take responsiblilty and accountability for managing caseloads for treatments and referrals.
