KEY RESULT AREAS Expert clinical practice To be lead Clinical Specialist Dietitian responsible for the Nutrition and Dietetic Service delivery to the adult Diabetes Virtual Ward and to manage a caseload with a high degree of complexity and without direct supervision whilst exercising a large degree of autonomy. To undertake complex nutritional assessments of patients independently as part of a multi-disciplinary team. To develop dietetic care plans for patients after calculation of nutritional requirements based on interpretation of biochemistry, anthropometry, anatomy and physiology, clinical condition/prognosis and other physical parameters when providing a clinical specialist nutrition service to adults with diabetes and undertake subsequent clinical monitoring of patients. To use advanced skills and extensive clinical and biochemical knowledge to diagnose malnutrition, dietary non-compliance, mineral and electrolyte abnormalities or metabolic crisis which require dietetic intervention and to independently provide and instigate a nutritional and dietetic plan. To make complex clinical decisions regarding necessary changes in the nutritional support provided e.g. oral nutritional supplements and enteral tube feeding nutrition To take a lead in clinical supervision and CPD activities within the Dietetic and Nutrition service. To be an expert user of anthropometric equipment and enteral feeding pumps and to train others to become competent in the use of such equipment. To lead on implementation of new nutrition products providing evidence for their use including financial implications/ benefits. To ensure that appropriate and evidence-based specialist advice on nutrition and dietetic issues is available to patients, clients and health professionals, within the remit of the post holders clinical duties. To practice and achieve the professional standards, protocols and policies set by the Dietetic and Nutrition Service and the wider Trust. Be responsible for service developments and develop policies, guidelines and procedures and supervise others in this development. This will include updating and development of nutrition and diet literature and resources, feeding plans and to ensure this is non biased and evidence based /best practice. To provide dietetic and nutrition input into all areas of adult diabetes making clinical decisions as necessary. To attend multi-professional meetings in order to provide expert nutrition and diet advice regarding of diet therapy and nutritional care of patients based on clinical evidence.
