Clinical Practice Provide advanced clinical assessment, diagnosis, and treatment for patients in care homes and those identified as frail in the community. Undertake proactive frailty reviews and personalised care planning in collaboration with patients, families, and the multidisciplinary team (MDT). Conduct regular ward rounds in allocated care homes, ensuring continuity and quality of care. Prescribe and review medications safely in line with best practice and prescribing guidelines. Recognise and respond to the acutely unwell patient, initiating timely interventions. Support end-of-life care planning, advance care plans, and DNACPR discussions where appropriate. Utilise and interpret appropriate investigations to inform decision-making. Leadership & Service Development Lead on the development, implementation, and evaluation of the practices care home and frailty strategy. Act as the key liaison between the practice, care homes, community services, and secondary care. Champion evidence-based practice and contribute to quality improvement initiatives. Provide mentorship, advice, and clinical support to colleagues and students. Contribute to the development of practice policies and protocols related to frailty and care home services. Education & Training Provide training and support to care home staff in clinical skills, frailty management, and recognising deterioration. Maintain and develop own clinical expertise through CPD, reflection, and peer review. Participate in clinical supervision and appraisal processes. Professional Responsibilities Work within the scope of the NMC Code and relevant advanced practice frameworks. Maintain accurate, timely, and complete patient records in line with legal and professional standards. Adhere to safeguarding procedures for vulnerable adults and children. Uphold confidentiality at all times.
