The Role

Job Title: Senior Support Worker

Hours: 37.5 hours per week

Shift Pattern: Monday to Friday, 9:00am to 5:00pm

We have an exciting opportunity for a Senior Support Worker to join our Hospital Discharge team in Wolverhampton.

The P3 Hospital Discharge Service is a single point of contact for homeless patients admitted to acute hospitals across the Black Country. Operating across Walsall, Sandwell, Dudley, and Wolverhampton, the service provides consistent and effective support from hospital admission through to discharge.

As a Senior Support Worker, you will play a key role in supporting the Service Manager in the day-to-day running of the Hospital Discharge Service. You will act as a deputy when required and serve as a central point of contact for the team, external professionals, and commissioners.

Working closely with support workers, you will review ongoing cases and provide guidance on interventions for individuals facing mental health challenges or barriers to accessing services. In addition, you will deliver face-to-face support, including discussions around non-clinical strategies to help individuals manage their wellbeing after discharge from hospital.
