Everyday people with chest, heart and stroke conditions are leaving hospital scared and alone. You can be part of our mission to make sure that there is no life half lived in Scotland. By joining Chest Heart and Stroke Scotland (CHSS) as Partnership & Relationship Manager you can be the difference between people just surviving and really living. You will be part of Scotland’s leading health charity providing support to people with chest, heart and stroke conditions and Long Covid to live life to the full again. Our Community Healthcare Support Service provides access to a range of supported self management and community recovery services to support people across Scotland to manage their health as well as possible and adjust to life after a diagnosis of a chest or heart condition or after a stroke. The purpose of this role is to support the Head of External Engagement, Peer and Participation, and colleagues across the External Engagement and Service Development Directorate, in building a strong and wide-reaching network of stakeholders. These relationships will help drive forward CHSS’s No Life Half Lived ambitions and highlight the value of our community healthcare support model in achieving shared strategic and operational goals across Scotland. You’ll play a key role in promoting and championing CHSS’s approach, helping to strengthen our reputation, broaden our reach, and demonstrate the impact of our work. This includes developing meaningful partnerships and engaging with influential decision-makers across the third sector, NHS, Scottish Parliament, NHS Boards, Integrated Joint Boards, and Health and Social Care Partnerships to increase our service user referrals. A particular focus of the role will be on identifying and developing strategic and operational partnerships in respiratory and cardiac health, increasing referrals and ensuring people living with these conditions receive the support they need.
