This is an excellent opportunity to join an outstanding rated trust as a full time Learning Disability Psychiatrist working in the Poole Community Learning Disability Team within the wider Bournemouth, Christchurch and Poole ‘BCP’ multidisciplinary team for adults with LD.
The team provides multidisciplinary community-based services to adult patients with Learning and Intellectual Disabilities registered with general practitioners within the area. You will be one of 3 psychiatrists in the BCP CLDTs.
You will provide a community psychiatry service for adult patients with Learning Disabilities in Poole. The total population of Poole is approx. 150,00 and the current psychiatry caseload is 65 adults, the current Poole CLDT caseload is 210.
Specialist epilepsy management for adults with LD is not part of the psychiatrist’s role in Dorset as this is provided by the consultant Neurology led Dorset Epilepsy Service, provided by University Hospitals Dorset NHS Foundation Trust ‘UHD’.
The Intensive Support Team /IST has a small base in Dorchester and their main base in Delphwood, Poole where the base for this post is.
The Intensive Support Team provides intensive support for individuals we support with a learning disability where the CLDT needs additional support. This service is provided in their own homes; residential or supported living; NHS acute beds, including adult mental health beds, and independent hospitals, including out of area placements. A key aim is to support the CLDTs to avoid unnecessary hospital admission and review those who are admitted to reduce admission length and facilitate discharge.
DHC also has a nurse-led Dorset-wide LD Intensive Support Team (IST) which supports the 9 Dorset CLDTs, and you will regularly attend their weekly online meetings.
You will provide LD Psychiatry for the Poole area of the Bournemouth, Christchurch, Poole (BCP) MDT and into the Intensive Support Team for Poole patients open to them. This includes new assessments and ongoing patient reviews, which will be by a mix of clinic and community visits. You will contribute to wider aspects of good clinical care such as risk assessments, management plans, appropriate referrals to colleagues, assisting with safe transfers of care when patients move CLDT patches, are admitted or discharged, relevant capacity assessments etc. You will engage with and contribute to team meetings, multiprofessional meetings, multiagency risk management meetings, CPA meetings, discharge planning meetings and Care and Treatment Review meetings etc.
Poole is the 2nd largest town in Dorset after neighbouring Bournemouth and is a desirable and well-connected location with direct trains to London, 10 miles from Bournemouth Airport and has a beautiful harbour on the same coastline as Dorset World Heritage Jurassic Coast.
