See full job description attached Provide specialist assessment by applying appropriate, evidence-based assessment approaches to the understanding and response to individuals needs Advise on the care of children and young people who have mental health needs and their carers/families Onward referral and liaison with appropriate community specialist services Communicate with the community teams to support them in reaching into the hospital for those patients who are open to them or if a new referral has been made to start the therapeutic relationship with them and be part of discharge planning.
