Equipping patients and their families with simple, confidence-boosting techniques and advice Supporting people through transitionsincluding loss of function and changes to goalswith care and clarity. Working as part of something bigger Being an active part of the hospices multidisciplinary teamthis means shared decisions, joint assessments, and respectful challenge Contributing to conversations that shape patient care, from early planning to end-of-life support Signposting and linking-in with external services that add value for our patients and carers Supporting the development of new services for a variety of patient groups, in line with Hospice strategy Maintaining effective relationships with external professionals and relevant organisations; liaising about patient care issues as appropriate, acting as a resource for the boroughs on hospice-based rehabilitation and use appropriate communication routes to share best practice Participating in appropriate rehabilitation forums in order to disseminate and share good practice and ensure service development. Communicating Developing positive working relations with members of the Hospital, Hospice, Social Care and Primary Healthcare Teams Participating in Multidisciplinary meetings relating to the care of patients and their families/ carers as required in the hospice/ hospital/ community setting Promoting and practicing excellent communication skills with patients, family and members of the multi-disciplinary team Contributing positively to the development of The Community Hospice as an anti-discriminatory organisation, actively promoting equality and diversity Quality Maintaining standards of care and reporting any concerns about care through Hospice clinical governance structures Participating in clinical reviews of patient and family care as appropriate Within the multidisciplinary team, working to ensure that patient dignity is respected at all times both before and after death, including acting as the patients advocate Participating in Hospice forums for discussion of research and audit programmes, including contributing to practice based audit to measure the outcomes of physiotherapy practice and impact of the role on patients Actively participating in research and contribution to evidence based practice in the clinical area Contributing to service development and evaluations as required Grief & Loss Supporting staff, students, patients and families in managing lossFurther information To be familiar with and adhere to all Hospice (and relevant Care Homes, Trust and Community) policies and procedures To support the ethos of the Hospice, supporting events and promotional activities as requested. You may be required to work in and from any Hospice or associated Company premises.
