In this role you will work with a high degree of autonomy, demonstrating core capabilities in clinical competency. You will treat patients with acute exacerbations of chronic diseases and acute presentations in the assessment, planning, and treatment of patients to prevent them being admitted to hospital. You will have the ability to triage and prioritise the needs of patients, making any referrals for further investigations where appropriate in line with local and trust guidelines, whilst ensuring your clinical practice is safe and effective, escalating appropriately to senior colleagues when needed. You should have excellent clinical, written, and verbal communication skills, be able to work independently and manage your own workload. The job role is community based across Boroughs of Lambeth and Southwark and works collaboratively with Primary, secondary and emergency care providers. The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working within agreed protocols. The post holder will deliver, implement, monitor, and escalate patients on the Evelina Childrens at Home virtual ward and where appropriate liaising with medical colleagues in order to assess, implement and modify treatment or to facilitate timely reassessment of patients on the service. They will ensure appropriate pathways of care and communication via liaison and referral to other agencies and specialist teams via the MDT as required. The post Holder will ensure compliance with Professional policies, National and local guidelines and Trust policies, procedures, and protocols ensuring they work within their scope of practice and clinical competencies. The post holder will also play a key role in the delivery and development of formal and informal education to members of the multi professional team. Will utilise advanced knowledge and skills and provide appropriate care, clinical reasoning skills and evidence-based practice to provide specialist advice and support for individuals or the multi professional team in the care of patients within the service. Work as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience. Our current working hours are 7 days a week, 0800-2200 for on a shift basis. Car driver essential. Pool cars (inc automatic) areprovided for team use. Clinical and Patient CareIndependently manage a group of patients within the specialty whilstworking as part of the multidisciplinary team, delivering specialisedindividualised and personalised direct patient care. Be responsible for assessing and recognising emergencies in thespeciality. To interpret information and take appropriate action. Demonstrate appropriate practice and specialist advice in the assessment,planning, implementation and evaluation of patient care. This includeshistory taking, physical examination and the requesting of agreeddiagnostic procedures/clinical investigations. Responsible for the correct administration of prescribed medication to ensure safe custody of medications in the home setting. Be responsible for assessing and recognising emergencies in thespeciality. To interpret information and take appropriate action. Act as an advocate for patients, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines. Ensure that patients receive high quality clinical care and a good patientexperience, having regard for their customs, religious beliefs anddoctrines. Recognise and avoid situations that may be detrimental to the health andwellbeing of the individuals. Recognise and monitor acute exacerbations of illness and initiateappropriate nursing interventions. Is wholly accountable for his / her practice in line with the NMC code ofprofessional conduct and takes every reasonable opportunity to sustainand improve his / her knowledge and professional competence.
