Crynodeb
The Advanced Clinical Practitioner will be primarily based within the PCN Enhanced Access Hub, working alongside other ACPs, ANPs, GPs and other health professionals, delivering care to PCN patients, including home visits. The post holder will: provide advanced clinical skills for patients attending the service. have at least 1 years Advanced Clinical Practice experience be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies and guidelines and contribute to service development to ensure patients have a seamless pathway. be expected to practice autonomously, formulate clinical decisions and treatment pans within local guidelines/policy to manage both acute and chronic illness. The post holder will require excellent communication skills which should include communicating clearly, sensitively and effectively with patients, families and professionals. Be able to signpost patients to the most appropriate services and be enthusiastic to apply acquired knowledge and skills. The post-holder will have a positive can do attitude, working with colleagues to deliver safe, kind and effective care.
