JOB SUMMARY: Working as an autonomous Advanced Nurse Practitioner as part of Greasby Surgerys team of doctors and nurses you will be expected to assess, triage and treat patients independently within the GP setting. The post holder will be a clinical expert in direct contact care and potentially telephone triage for patients who may need urgent (on the day) treatment or advice for a range of conditions. KEY DUTIES AND RESPONSIBILITIES: Working with the team of doctors and nurses at Greasby Surgery the post holder will be responsible for:- Clinical/Job Skills Undertake the assessment, evaluation and planning of patient care packages. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based and national and practice protocols, and within scope of practice. Analyse signs and symptoms, laboratory tests and other measures of function to formulate a diagnosis. Identify and assess any complication surrounding patient care and refer to physicians as appropriate. Utilise sound clinical decision making to provide evidence based treatment plans in response to clinical assessment. Ensure high QOF and QIF achievements and other local enhanced incentives. Monitor indicators of chronic disease, anticipating possible decline, and proactively manage to enhance well-being and maintain independence. Manage caseload and integrate care across all settings preventing duplication, fragmentation and delay occurring as patients move between care settings. Develop specialised care plans for each patient on caseload. Empower patients and carers to evaluate possible treatment options as disease progresses and evaluate relative benefits and burdens of these options. Plan interventions in light of patients expressed wishes and goals of care, facilitating discussion with the family and significant others concerning end of life care plans. Communication To be able to maintain accurate electronic and written patient records. Attend team meetings or other Practice meetings as required. Negotiate with professionals in all settings to provide care in the most appropriate and least invasive manner possible. Provide comprehensive training sessions to team members and other health professionals as required. Other Responsibilities The responsibility of the post will change over time and will be subject to ongoing review between the post holder and line manager. The post holder may be required to undertake other duties to meet the needs of this developing service. - Adhere to organisational policies - Information Governance - GDPR - Records management - Caldicott - Audit and Effectiveness - Data Returns - Participation / attendance at meetings - Maintenance of active NMC registration - Maintenance of active Indemnity Practice staff are expected to adhere to the smooth and effective running of the Practice and will be expected to work as part of the Team. Personal / Professional Development The post-holder will participate in any training programme implemented by the Practice as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice Policies and Procedures relating to confidentiality and the protection of personal and sensitive data. Quality The post-holder will strive to maintain quality within the practice, and will: Alert other team members to issues of quality and risk. Assess own performance and take accountability for own actions, either directly or under supervision. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Work effectively with individuals in other agencies to meet patients needs. Effectively manage own time, workload and resources. Communication The post-holder should recognise the importance of effective communication within the team and will strive to: Communicate effectively with other team members. Communicate effectively with patients and carers. Recognise peoples needs for alternative methods of communication and respond accordingly. Contribution to the Implementation of Services The post-holder will: Apply Practice Policies, Standards and Guidance. Discuss with other members of the team how the Policies, Standards and Guidelines will affect own work. Participate in audit where appropriate.
