Crynodeb
Direct contact with patients if undertaking a investigation, by telephone or face to face. Involved with patient in relation to patient safety walkabout in clinical areas and patient safety clinical audit providers. Will act as a source of expert advice, drawing upon extensive clinical experience in a variety of setting and knowledge of patient clinical outcomes, patient safety and quality improvement initiatives. To advise provider organisations on appropriate standards and requirements for determining a Primary Health Need and to assist in the development of competent assessors within the organisation To communicate with national and local organisations, public bodies, patients and carers and independent providers on complex sensitive matters and issues in complex situations arising from the application of NHS Continuing Healthcare criteria and the outcome. To work in an advisory capacity across the DGFT Commissioning teams to facilitate the implementation of PHBs and the development of functional operating procedures to support this implementation. Manage unpredictable and challenging situations; acting in a sensitive manner when communicating to patients and/ or their representatives, decisions in relation to eligibility both verbally and in writing, recognising that imparting this information may include settings that are high emotive, emotional and challenging e.g. situations such as at end of life.
