More detail about the role Based within the Bideford Community Rehabilitation Team, youll work across a beautiful coastal and rural area of North Devon. We deliver high-quality, person-centred rehabilitation in patients homes, as well as in nursing and residential settings. Youll be part of a highly collaborative multidisciplinary team, working closely with GPs, community nurses, social care, and support staff across four local GP practices. Our integrated working environment supports shared learning, fast communication and continuity of care.
