Summary

Key Responsibilities: Safeguarding (Children & Adults): Act as the administrative point of contact for all safeguarding concerns and documentation. Maintain accurate safeguarding registers and case records. Organise, minute, and follow up actions from safeguarding meetings. Support the timely sharing of information with local safeguarding partners, in line with confidentiality and data protection regulations. Ensure all staff safeguarding training records are up to date. Work closely with the Safeguarding Lead GP and Practice Manager to ensure CQC compliance and policy implementation. Palliative and End-of-Life Care: Maintain and regularly update palliative care registers. Coordinate monthly Gold Standards Framework (GSF) or MDT palliative care meetings including agendas, documentation, and follow-up actions. Liaise with hospices, district nurses, community services and carers to ensure care plans are shared and implemented. Monitor and support implementation of personalised care plans and anticipatory care arrangements (e.g., DNACPRs, ReSPECT forms). Track and monitor progress of referrals, ensuring timely communication between healthcare professionals and patients/families. General Coordination & Administration: Liaise with patients, families, and carers to support their understanding of care pathways and ensure continuity of care. Support audit and reporting requirements related to safeguarding and palliative care. Maintain up-to-date knowledge of local pathways, services, and referral criteria. Provide general administrative support to the care navigation and clinical teams where needed. Create weekly MDT agenda, minute meeting and track actions
