Role Overview: As a Practice Nurse, you'll play a vital role in the delivery of chronic disease management and general treatment room services. A particular interest or specialism in diabetes is highly desirable, though a broad knowledge across all chronic conditions (including Asthma, COPD, and CHD) is preferred. Training and support will be availableand expectedwhere upskilling is required. Key Responsibilities: Health promotion and patient education Chronic disease management (including Diabetes, Asthma, COPD, CHD) Childhood immunisations Cervical cytology and smear tests Supporting Enhanced Access sessions as part of our extended hours offering
