You'll lead the Rehabilitation and Wellbeing Team in delivering high quality palliative rehabilitation and wellbeing support to patients and carers. This isn't just about care its about helping people live fully, with dignity, for as long as they can. You'll bring a holistic approach to support people to live as well as they can for as long as they can. You'll work alongside colleagues from a range of disciplines within the hospice on the inpatient unit, outpatient and community services and advise the hospices hospital-based team. Sharing your expertise to help us all deliver the best care we can. You'll build strong, honest relationships with partners across the health and care system because great end-of-life care doesn't happen in isolation. Key Responsibilities: Clinical Practice & Leadership You'll work with the team to deliver care that is as individual as the people we support, always holistic and person centred. As the Lead for the Rehabilitation and Wellbeing Service, you will be a key member of the team, providing evidenced based clinical expertise and leadership and will: Lead and champion the service making sure it is seen, heard and valued You'll build strong, honest partnerships with colleagues working in other services because great care doesn't stop at the hospice You'll lead your team with clarity and care- supporting their growth, guiding their work and making sure they feel confident and capable every step of the way You'll help people hold on to what matters mobility, independence, dignity, social networks by creating care plans that are practical, empowering and personalised You'll lead by example showing what compassionate, skilled care looks like and lifting others along the way You'll stay curious, thoughtful and grounded in evidence always asking, is this the best we can do? You'll work closely with senior colleagues to shape and grow the service, bringing your experience and ideas to the table You'll help make the case for funding by sharing the impact of your work, stories, outcomes and the difference we make You'll help build a brilliant team recruiting people who share our values and shaping roles that make a real difference You'll be an advocate for patients, ensuring that dignity is respected before, during and after death because everyone deserves to be seen, heard and respected You'll meet people where they are literally by visiting them at home, assessing needs and making sure the right support is in place, liaising with the integrated neighbourhood team You'll work with the team to help patients set meaningful goals and navigate their journey making sure they're supported every step of the way, even after they leave our care You'll share what works, learn from others and help shape the future of palliative rehabilitation and wellbeing, taking part in locality and network forums You'll stay grounded in your professional standards and code because safe, ethical care is non-negotiable You'll help us to make the best use of our resources so we can reach more people and do more of what matters You'll make sure that our equipment is in top shape, and that the records reflect this When things go wrong, you'll face it head on learning, improving and making sure we do better next time, participating in investigations of incidents, concerns and complaints when required You'll help shape the big picture bringing your voice and vision to our future plans Communication You'll help shape a team culture that's open, adaptable and grounded in our values where people feel safe to speak up, collaborate and support one another You'll connect people, bringing people together across teams and services to make sure patients get joined up, responsive, compassionate care. You'll ensure our rehab and wellbeing service is visible, valued and trusted. You'll keep records that are clear, timely and meaningful because good notes mean better care You'll make the most of digital tools to share insight and improve care accessibility and quality You'll share your expertise in team meetings and make sure the voice of rehab and wellbeing is still heard when you cant be there You'll communicate with honesty, warmth and clarity whether you're talking to a patient, a family member or a colleague You'll make sure patients and families have the information they need no jargon, keeping it simple, the truth delivered with care You'll stand up for inclusion and fairness, taking an actively anti-discriminatory approach, helping us build a hospice where everyone feels welcome, respected and seen. Quality You'll keep standards high and when somethings not right, you'll speak up and help fix it. That's how we all stay accountable to the people we care for You'll use data to tell the story of our impact what's working, what needs to change and how we can keep improving sharing this through our governance and assurance processes You'll help shape the way we work writing clear guidelines and protocols and leading audits or collaborative research to move us forward Grief & Loss You'll take care of yourself and be there for people staff, volunteers, patients and families when they are facing the hardest moments. Offering steady compassionate support when its needed most You'll spot when someone is (likely to struggle) struggling with grief before or after a loss and make sure they get the support they need. Education & Training You'll stay curious and committed to learning and you'll encourage others to do the same. Whether its a course, study day or a shared moment in practise, you'll help turn everyday work into an opportunity for growth You'll help shape our training plans sharing the needs of your department to ensure we've got the tools and budget to support development You'll build a culture where learning is part of the job not an extra. Questions are welcome, mistakes are shared and everyone's encourages to grow You'll make sure the essentials are covered keeping yourself and your team up to date with all mandatory training You'll share what you know whether that's teaching colleagues in the hospice or contributing to wider training programmes. You'll encourage and welcome students and visiting professionals into the team offering guidance, support and insight into what great palliative rehabilitation looks like. Further information You will be registered with the Health Care Professions Council Youll support the ethos of the Hospice, supporting events and promotional activities as requested to help us reach more people who need us Youll follow our policies to keep our care safe, respectful and consistent (and relevant Care Homes, Trust and Community) policies and procedures. Youll treat peoples information with the same care and respect you give to them, in line with the requirements of the Data Protection Act Youll help keep our spaces safe - for pateints, families, staff and volunteers by staying alert and speaking up when somethings not right, in line with the Health and Safety at Work Act and policies to identify and report, any untoward accident, incident or potentially hazardous environment. Youll be flexible, because we may require you to work in and from any Hospice or associated Company premises. This role will evolve as we do. Your job description is intended as an outline of the general areas of activity and may be amended from time to time in the light of the changing needs of the hospice. It will be reviewed with you as the service grows and changes
