Job Summary: We are seeking a dynamic and experienced Registered Nurse or Paramedic to join our team working in the Emergency Department in a Streaming Role. This vital position involves initial assessment of walk-in patients, directing them to the most appropriate care pathway, and ensuring timely access to appropriate services in line with clinical urgency. As a first point of clinical contact, you will play a key role in improving patient flow, reducing waiting times, and enhancing patient experience by supporting safe, effective streaming and redirection to suitable services including Urgent Care, General Practice (GP), or Emergency Department (ED) services, Pharmacy, Community Urgent Eye Service (CUES). Key Responsibilities: Perform timely clinical assessment (streaming) of patients presenting to Urgent and Emergency Care (UEC) settings. Use clinical judgement to identify red flags and signs of serious illness or injury. Make autonomous decisions on directing patients to the correct clinical area or service (e.g., ED, urgent treatment centre, GP out-of-hours). Escalate appropriately to senior clinicians in complex or high-risk presentations. Maintain accurate and concise records of all assessments and clinical decisions. Collaborate with multidisciplinary teams to ensure smooth and effective patient flow. Promote a safe environment by adhering to infection control, safeguarding, and risk assessment procedures. Support service improvements and contribute to audits, training, and development initiatives. Requirements: Essential: Registered Nurse (RGN) with NMC registration or Paramedic registered with HCPC. Significant post-registration experience in urgent, emergency, or pre-hospital care. Strong clinical assessment, triage, and decision-making skills. Ability to work autonomously in a fast-paced environment. Excellent communication and interpersonal skills. Proficient IT and documentation abilities. Desirable: Experience in clinical streaming, triage, or primary care settings. Advanced Life Support (ALS) or equivalent certification. Evidence of continued professional development (e.g., minor injury/minor illness course). Knowledge of local urgent care pathways and referral criteria. Working Conditions: Shift-based role including weekends, evenings, and bank holidays. Fast-paced, high-pressure clinical environment requiring resilience and adaptability.
