Crynodeb
The role of the Gloucester House CAMHS Nurse will be to provide CAMHS nursing input to young people aged between 5-14 years.This will involve intensive working as part of an MDT Team.
You will be responsible for a defined caseload of clients, delivering a range of evidence-based interventions for pupils with social emotional and mental health needs with co-occurring neurodiverse presentations. You will work in an integrated way with education within a multi-disciplinary team and will receive regular clinical supervision from a senior member of the Service. You will also be expected to work jointly with other members of the team offering specialist CAMHS advice, consultation and training as required.
You will have a keen interest in improving outcomes for young people with SEMH and neurodiverse conditions as well as capturing these within the caseload work. Training will be provided alongside access to the training and CPD opportunities working at the Tavistock and Portman enables.
· Identify and support diagnosis of neurodiverse conditions, nursing related presentations relating to medication side effects, depression and anxiety, and other health related issues
· Support the planning, implementation andevaluationof programmesofevidenced-basedcaretoserviceusers, conscious of their changing healthcare needs and varying levels of complexity, working collaboratively with other clinicians and stakeholders
· Liaise with MDT and wider networks including FCAMHS, CAMHS, school nursing and psychiatry
· Identify and assess risk and complete and update necessary documentation
· Participateinthecareplanningprocessundersupervisionwithinthecontextof multidisciplinary team including case reviews.
· To carry out on-going assessment of the impact of prescribed medication on individual young people both in terms therapeutic effect and untoward side effects.
· Communicate in a competent and sensitive manner, complex information concerningtheassessment,formulationandtreatmentplansofclientsunder their care where clients may be volatile and aggressive and/or resistant totreatment.
· Participateincaseconferences,reviewandplanningmeetingsincludingwith external agencies as appropriate.
