Summary
We have an exciting opportunity for a fixed term 12 month position - Care Co-ordinator - Nurse Liasion.
• 1 x 18.75 hours fixed term 12 months.
Care co-ordinators play an important role within a practice to proactively identify and work with patients. The Nurse Liaison role will be fundamental in supporting the Practice Nursing Team globally across Royal Primary Care. They work closely with the Practice Nursing teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to people and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
Key responsibilities
Patient Assessment: Evaluating the healthcare needs of patients and ensuring they receive appropriate care.
Coordination of Care:Collaborating with various healthcare professionals to create a cohesive care plan and completing referrals as needed.
Education:Providing patients and their families with information about medical conditions, treatments, and available resources. · Advocacy: Acting as a patient advocate to ensure their needs and preferences are respected. · Follow-Up: Monitoring patient progress and adjusting care plans as necessary.

Other responsibilities within the role;
• Work with people, their families and carers, to improve their understanding of their condition.
• Support people to develop and review personalised care and support plans to manage their needs and achieve better healthcare outcomes.
• Help people to manage their needs by providing a contact to answer queries, make and manage appointments, and ensure that people have good quality written or verbal information to help them make choices about their care.
• Assist people to access self-management education courses, peer support, health coaching and other interventions that support them in their health and wellbeing, and increase their levels of knowledge, skills and confidence in managing their health.
• Provide co-ordination and navigation for people and their carers across health and care services. Helping to ensure patients receive a joined-up service and the appropriate support from the right person at the right time.
• Work collaboratively with all primary care professionals within the surgery to proactively identify and manage a caseload, which will include patients with long-term health conditions, and where appropriate, refer back to other health professionals within the surgery.
• Raise awareness of how to identify patients who may benefit from shared decision making and support surgery colleagues and people to be more prepared to have shared decision-making conversations.
• Support the practice in developing communication channels between GPs, clinical teams, people and their families and carers and other agencies.
• Identify carers and help them access services to support them liaising with the practice carers champion
• Conduct follow-ups on communications from out of hospital and in-patient services.
• Maintain records of referrals and interventions to enable monitoring and evaluation of the service.
