TheAdvanced Clinical Practitioner (ACP)is a highly skilled healthcare professional responsible for delivering expert, autonomous patient care in a primary care setting. The ACP will assess, diagnose, treat, and manage patients with acute and chronic conditions, working collaboratively with GPs and the wider multidisciplinary team.
Key Responsibilities:
• Conduct clinical assessments and make independent treatment decisions.
• Diagnose and manage long-term and urgent conditions.
• Prescribe medications and initiate treatment plans.
• Support service development and clinical governance.
• Provide leadership, education, and mentorship to junior staff.
1. Clinical Assessment & Diagnosis
• Conduct comprehensive patient assessments, including history-taking and physical examinations.
• Diagnose and manage acute and chronic conditions, initiating appropriate treatment plans.
2. Treatment & Prescribing
• Prescribe medications (if qualified with V300) and initiate treatments within the scope of practice.
• Perform clinical procedures and interventions as required.
3. Patient Management & Care Planning
• Develop, implement, and evaluate personalized care plans.
• Provide follow-up care and refer patients to appropriate specialists when necessary.
4. Multidisciplinary Team Collaboration
• Work closely with GPs, nurses, paramedics, and other healthcare professionals to ensure integrated patient care.
• Support and contribute to the development of primary care services.
5. Clinical Governance & Service Improvement
• Participate in audits, quality improvement projects, and risk management activities.
• Ensure compliance with clinical guidelines and best practices.
6. Leadership, Teaching & Mentorship
• Supervise and mentor junior staff, trainee ACPs, and students.
• Lead training sessions and contribute to staff development.
7. Emergency & Urgent Care
• Provide rapid assessment and management of urgent cases.
8. Documentation & Record-Keeping
• Maintain accurate, clear, and concise patient records in line with legal and professional requirements.

• Perform advanced clinical interventions and procedures within their competence.
• Lead and support multidisciplinary teams in delivering high-quality patient care.
• Work collaboratively with GPs, nurses, and other healthcare professionals to enhance service efficiency.
• Contribute to clinical governance, quality improvement, and patient safety initiatives.
• Educate and support junior staff, students, and trainee ACPs.
Qualifications and Skills:
• Registered healthcare professional (e.g., nurse, paramedic, pharmacist, physiotherapist) with a relevant master’s-level qualification in advanced practice.
• Independent prescribing qualification (V300).
• Extensive clinical experience with evidence of autonomous decision-making.
• Strong leadership, problem-solving, and communication skills.
