Complete comprehensive assessments and reviews for service users who meet the eligibility criteria for NHS funded Tertiary and Continuing Health Care. To support the care co-ordinator to develop documentation for presentation to CCG commissioners are required - setting timelines, monitoring completion. Attending MDT meeting, relevant panels, preparing documentation and updates for review. Where appropriate, scoping alternative personalised options, including consideration of mainstream provision that will meet the recovery needs of the individual. This will involve developing new packages of care, costing up of alternative option and presentation to the Ensure service users, carers and advocates are involved in the assessment and review process regarding care needs, and involve them in the decision-making process. Work with the local/placement teams to implement a variety of appropriate placement measures which may include returning service users from out of borough placements to their own locality. Proactively negotiate with providers regarding costings which reflect efficiencies in the system and the needs for treatment/intervention of the individual. Promote high quality service provision, social inclusion, personalisation and positive outcomes for service users Organise work plans and provide assurance that efficiency targets will be met Communication: Develop and maintain effective and positive communication and working relationships with all service users, carers and professionals Utilise appropriate communication methods in order to engage with service users experiencing barriers to their understanding (eg severe psychosis, sensory impairment, unfamiliar terminology or processes) Lead in problem solving and finding solutions where conflict exists in relation to decision making about the most appropriate care for service users Offer support and advice to carers and advocates to ensure they understand and if appropriate are invited to participate in the continuing care and transfer process. Develop relationships with local providers, both statutory and non-statutory, to ascertain if service user needs could be met within local service provision rather than out of borough placements Promote effective liaison and operational links for local teams with the providers Carry out detailed assessments and provide reports to assist with transfer to suitable step-down placements and present to the appropriate funding and placement panels Establish effective communication with healthcare professionals, social services and primary healthcare teams in order to ensure continuity of care and support effective implementation of transition plans Maintain accurate written and IT communication systems Undertake Participate in audit and monitoring of providers This is an outline of the post holders duties and responsibilities.
