Summary


This is an exciting role , which involves ensuring that all SLaM funded mental health residential, nursing, hospital, Rehabilitation , Private and Block placements are rigorously reviewed within a Recovery/promotion of independence model.

One SLaM OT to joint work with a SLaM nurse and the Move On Support Team (Southwark Local Authority) to always focus on promoting service-users independence, stepping down or increasing care provision as appropriate.

To challenge placements which do not meet service user's needs, using the review process to provide clear outcome focused goals for step up/down.

In the first instance focus on high cost placements which may be external to the London Borough of Southwark, requiring travel and liaison with other NHS trust providers.

To ensure that all relevant professionals, carers, friends, and families are consulted and all relevant issues, for example, the service user's capacity are taken into account, as a team.

Complete comprehensive placement reviews for service users who meet the eligibility criteria.

Support care co-ordinators to develop step down pathways, with personalised packages of care which support and build on people’s assets, and which may be an alternative to entry into national/specialist service, or may secure timely discharge.

To support the care co-ordinator & local teams to develop documentation for presentation to commissioners & local panels are required - setting timelines, monitoring completion.

Ensure service users, carers and advocates are involved in the assessment and review process regarding care needs, and involve them in the decision-making process.

Develop relationships with local providers, both statutory and non-statutory, to ascertain if service user needs could be met within local service provision rather than out of borough placements

Lead in problem solving and finding solutions where conflict exists in relation to decision making about the most appropriate care for service users.

South London and Maudsley NHS Foundation Trust (SLaM) provide the widest range of NHS mental health services in the UK as well as substance misuse services for people who are addicted to drugs and alcohol. We work closely with the Institute of Psychiatry, Psychology and Neuroscience (IoPPN), King's College London and are part of King's Health Partners Academic Health Sciences Centre. There are very few organisations in the world that have such wide-ranging capabilities working with mental illness.

SLaM employ around 5000 staff and serve a local population of 1.1 million people. We have more than 230 services including inpatient wards, outpatient and community services. Currently, provide inpatient care for approximately 5,300 people each year and treat more than 45,000 patients in the community in Croydon, Lambeth, Lewisham and Southwark; as well as substance misuse services for residents of Bexley, Bromley and Greenwich.

Complete comprehensive assessments and reviews for service users who meet the eligibility criteria for NHS funded Tertiary and Continuing Health Care. To support the care co-ordinator to develop documentation for presentation to CCG commissioners are required - setting timelines, monitoring completion.

Attending MDT meeting, relevant panels, preparing documentation and updates for review.

Where appropriate, scoping alternative personalised options, including consideration of mainstream provision that will meet the recovery needs of the individual. This will involve developing new packages of care, costing up of alternative option and presentation to the

Ensure service users, carers and advocates are involved in the assessment and review process regarding care needs, and involve them in the decision-making process.

Work with the local/placement teams to implement a variety of appropriate placement measures which may include returning service users from out of borough placements to their own locality.

Proactively negotiate with providers regarding costing’s which reflect efficiencies in the system and the needs for treatment/intervention of the individual.

Promote high quality service provision, social inclusion, personalisation and positive outcomes for service users

Organise work plans and provide assurance that efficiency targets will be met

Communication:

Develop and maintain effective and positive communication and working relationships with all service users, carers and professionals

Utilise appropriate communication methods in order to engage with service users experiencing barriers to their understanding (eg severe psychosis, sensory impairment, unfamiliar terminology or processes)

Lead in problem solving and finding solutions where conflict exists in relation to decision making about the most appropriate care for service users

Offer support and advice to carers and advocates to ensure they understand and if appropriate are invited to participate in the continuing care and transfer process.

Develop relationships with local providers, both statutory and non-statutory, to ascertain if service user needs could be met within local service provision rather than out of borough placements

Promote effective liaison and operational links for local teams with the providers

Carry out detailed assessments and provide reports to assist with transfer to suitable step-down placements and present to the appropriate funding and placement panels

Establish effective communication with healthcare professionals, social services and primary healthcare teams in order to ensure continuity of care and support effective implementation of transition plans

Maintain accurate written and IT communication systems

Undertake Participate in audit and monitoring of providers
