Job Title: PCN Manager and Digital & Transformation Lead Job Type: Fixed Term to 31 March 2026 in line with Network Contract Directed Enhanced Service (DES) Specification. Hours of Work: Up to 37.5 hours per week Salary: Depends on experience Location: To be agreed but the postholder will primarily be home-based with travel to any Icknield Primary Care Network GP Practice premises. Reports to: PCN Clinical Directors Accountable to: PCN Clinical Directors Main Duties and Responsibilities Operational Management: - Leading the coordination and delivery of existing and new PCN projects, including those linked to the DES, IIF and Enhanced Access Service - Supporting workforce planning and management of ARRS roles - Ensuring the smooth day-to-day operation of the PCN including - governance, compliance, and contract management - Maintaining a good knowledge of key policies from relevant government departments and agencies, and support PCN staff to understand and interpret these locally Financial Management: - Overseeing budgets and funding streams related to PCN activity - Preparing financial reports in line with PCN objectives Stakeholder Engagement and Communication: - Building and maintaining effective relationships with local GP practices, Hertfordshire and West Essex ICB and other stakeholders - Representing the network at external meetings Data and Performance Monitoring: - Work in partnership with the ICS to identify relevant data sets and to support standardisation in the use of measures for understanding change and improvement in general practice. - Presenting and communicating data to practices and PCN staff at all levels to increase understanding of operational challenges - Interpreting data and highlighting variation to support open discussion about where variation is and is not warranted Digital and Transformation: - Working with Hertfordshire and West Essex ICS to support the development of overall primary care transformation strategies, including digital transformation strategies and their delivery - Supporting the PCN and member practices to adapt and apply specific policies and initiatives to local needs and to prioritise initiatives based on local requirements - Identifying best practice and learning from other areas that could be applicable to the local PCN and communicate this to staff. Developing and managing an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering the potential impacts on the wider organisation and determining resource requirements. - Supporting practices and staff to effectively use existing and new technology systems and products to enable improvement Administrative Oversight: - Providing administrative support to Clinical Directors and ensure the smooth running of PCN operations - Leading relevant working groups, meetings and other forums across the PCN to agree changes, share learning, discuss challenges and resolve issues Health and Safety: The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, to include: - Using personal security systems within the workplace according to practice guidelines. - Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks. - Making effective use of training to update knowledge and skills. - Adhere to all aspects of the Health & Safety at Work Act and Control of Substances Hazardous to Health (COSHH) regulations, ensuring good hygiene, cleanliness and a safe environment in the dispensary. - Reporting potential risks identified. Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the PCN as a business organisation. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data. Data Protection The Data Protection Act 2018 regulates the use of personal data. You are required to ensure that any personal information obtained, processed or held (on a computer or otherwise), is done so in a fair and lawful way and that the data held and processed is only for the specified registered purposes, in particular personal data relating to patients, or other persons, shall be disclosed to any person without the consent of the individual who is subject of the data or otherwise by lawful authority or justification. Standards The GP Practices aim to maintain the goodwill and confidence of its own staff and of the general public. To assist in achieving this objective it is essential that, at all times, employees carry out their duties in a courteous, sympathetic manner. Quality The post-holder will strive to maintain quality within the PCN, and will: - Alert other team members to issues of quality and risk. - Assess own performance and take accountability for own actions, either directly or under supervision. - Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. - Effectively manage own time, workload and resources to deliver effective medicines Communication The postholder should recognise the importance of effective communication within their team and will strive to: - Communicate effectively with other team members. - Communicate effectively with patients and carers. - Recognise peoples needs for alternative methods of communication and respond accordingly. Contribution to the Implementation of Services The post-holder will: - Apply Practice policies, standards and guidance. - Discuss with other members of the team how the policies, standards and guidelines will affect their own work. - Record data in the required manner and to a high standard - Participate in audits where appropriate. Training and Development The successful post holder will be expected to be responsible for his/her continuing professional development and to take a proactive approach to maintaining personal and professional effectiveness in an evolving role. Training will include mandatory training and role-specific training, as appropriate; all of which must be completed in a timely manner. The job specification may be amended following consultation with the postholder to facilitate the development of the role, the organisation and the individual. All members of staff should be prepared to accept additional duties, or surrender existing duties, to enable the efficient running of the organisation and to support change management processes to support the success of the organisation. Additional duties will not be outside your abilities, skills or experience.
