Full job description and person specification are included in the attached file. Working alongside nurses, AHPs, pharmacist, and case coordinators, you will be undertaking a range of proactive assessment and interventions to enhance health and wellbeing in this patient group, identifying actions which can be taken to prevent hospital admission, avoid unnecessary admission to care homes, and support patients to live well. This will be delivered in partnership with the patient's usual registered GP.
