Full details of the job description and service specification are in the attached document. The postholder will be an experienced and established clinical pharmacist with experience working with polypharmacy, structured medication reviews, medication optimisation. The patient cohort is likely to be clinically complex with multifactorial health and social care needs where a person centred and pragmatic approach is needed.
