Summary
We are seeking an experienced and motivated Senior Social Worker (Adults) with 5+ years of post-qualifying experience. This role is crucial in leading complex discharge planning and supporting reduced long lengths of stay (LLOS) for patients, particularly within a hospital discharge and community support context.
Essential Requirements:
Qualified Social Worker (degree in Social Work or equivalent)
Registered with Social Work England
Minimum 5 years’ post-qualifying experience in adult social care
Significant experience in hospital discharge and multi-agency working
Up-to-date knowledge of Hospital Discharge & Community Support Guidance
Eligible to work in the UK (proof required)
3 years’ references/work history (with explanations for any gaps)
Supporting documents must be provided upon offer acceptance
Must sign a Confidentiality Agreement upon offer
Valid Standard DBS required
