Summary
Provide clinical and strategic direction to ensure coherence and consistency across eight neighbourhoods Support PCNs to drive transformation by aligning neighbourhood priorities with operational and clinical goals Act as a senior system leader across borough forums, governance groups, and ICS transformation programmes Strengthen neighbourhood capabilities in data-driven planning, proactive care, and partnership working Facilitate cross-system collaboration between primary care, community services, the local authority, and VCSE partners Lead on clinical engagement, workforce development, and succession planning at neighbourhood level What were looking for: GMC-registered clinician with extensive clinical leadership experience at system or borough level In-depth understanding of PCN development, MDT collaboration, and neighbourhood-based transformation Ability to operate with credibility across strategic forums and civic partnerships Skilled in coaching, organisational development, and change facilitation Committed to reducing inequalities and embedding population health management approaches Familiarity with City and Hackney or comparable boroughs is desirable, as is experience using PHM tools and leading integrated initiatives.
