Crynodeb

Accurately add encounters and referrals for patients requiring secondary care. Communicate effectively with patients who may have difficulty expressing themselves, using de-escalation techniques when necessary. Provide VBT (Very Brief Advice) to current smokers, offering encouragement and support to quit smoking. Work collaboratively with multidisciplinary teams, sharing knowledge while respecting role boundaries. Line manage and support Patient Contact Assessors (PCAs) to ensure the team meets performance targets. Act as the first point of escalation for complex patient queries and team concerns. Assist in recruitment, training, and onboarding of new PCAs, ensuring they are well-equipped for their roles. Support the Performance and Pathway Manager and the Failsafe and Administration Manager in performance reviews, one-to-one meetings, and staff development. Monitor sickness absence and performance within the team, conducting return-to-work interviews and escalating concerns as required. Oversee team rotas and workload, adjusting as needed to maintain service delivery. Schedule and facilitate monthly team meetings, preparing agendas, ensuring productive discussions, following up on action items, and promoting effective communication and collaboration within the team. Approve and monitor annual leave requests, ensuring adequate team coverage, and maintaining effective scheduling to meet service needs.
