Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will play a pivotal role in ensuring that clients receive the highest quality of care while navigating their healthcare journey. The Care Coordinator will liaise with clients, families, and healthcare professionals to facilitate seamless communication and support.
Responsibilities
Coordinate care plans for clients, ensuring all necessary services are provided efficiently.
Act as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
Maintain accurate records of client interactions and care plans within the office system.
Collaborate with healthcare providers to ensure comprehensive care delivery.
Schedule appointments and follow-up visits for clients as needed.
Monitor client progress and adjust care plans in consultation with relevant stakeholders.
Provide administrative support within the office, including filing, data entry, and maintaining documentation.
· To provide leadership to care staff within the service
To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements and relevant regulations under the direction of the Manager
To supervise the care delivery within the service
To maintain skills at a current level, and undertake such training and development as may, from time-to-time, be required to maintain practices as up to date
Experience
Previous experience in a healthcare or social services setting is advantageous.
Strong organisational skills with a keen attention to detail.
Excellent communication skills, both verbal and written, to engage effectively with clients and professionals alike.
Proficiency in using office software and systems for record keeping and scheduling.
A compassionate approach to client care, demonstrating empathy and understanding in all interactions.
