Crynodeb
To provide high-quality, patient-centered physiotherapy services in the community, focusing on assessment, treatment, rehabilitation, and admission avoidance. The post holder will manage a varied caseload, contribute to service development, and support junior staff through supervision and clinical leadership. Clinical Responsibilities: Conduct comprehensive assessments of patients physical, functional, and psychosocial needs. Develop, implement, and review individualized treatment and rehabilitation plans to support patients in achieving their goals. Assess for and prescribe necessary equipment, aids, and adaptations to promote safety and independence. Provide treatment interventions in line with best clinical practice and patient-centered goals. Monitor patient progress, identify changes in condition, and escalate any signs of deterioration appropriately. Support safe and effective discharge planning, including from Frimley Park Hospital and other acute settings, aiming to reduce hospital admissions. Deliver crisis interventions and rapid response to patients at risk of admission.
