We are delighted to be able to advertise this position to become part of our integrated, multidisciplinary team covering North East Hampshire.
You will be working in the community, treating patients in their own homes across Fleet, Farnborough, Aldershot and Yateley.
You will be part of the nursing team, which forms part of our Enhanced Recovery and Support at Home team, along with OTs, Physios, APs, Rehab Support Workers and admin.
We are a rehab service, supporting patients at home either to prevent admission to hospital or on their discharge home. Referrals are usually from the wards or from the GP, Paramedic Practitioners, members of the ICT etc. In addition to holistic assessments, primary nursing needs include catheter care, wound care and medication management.
The Nursing Associate will:
• Be responsible for delivering high quality, patient centred, compassionate care under the direction of a Registered Nurse (or other registered care professional, dependent on setting) with a focus on promoting health and independence
• Have proficient attitudes and behaviours compatible with NHS Values
• Be an accountable professional
• Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual
• Carry out specific clinical and care tasks and responsibilities to a high standard and competency, under the direction of a registered nurse or other registered care professional dependent on setting
• Be able to work without direct supervision, at times delivering care independently in line with the individual’s defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed.
• The role is flexible and adaptable to the changing needs of the care setting, and it is expected that the responsibilities will be developed, and be agreed, as services evolve.

The following list is indicative: tasks and responsibilities will vary depending on the care setting the Nursing Associate is working in.
The Nursing Associate will:
• Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the on-going assessment, planning, management and evaluation of care
• Provide and monitor care
• Support individuals with all aspects of care including personal care, daily living, providing person centred care, and promoting health and independence through awareness raising and care navigation to other professionals as appropriate
• Perform and record clinical observations including blood pressure, temperature, respirations, pulse
• Be prepared to undertake additional skills once competent including but not exhaustive; cannulation, venepuncture, ECGs as per training and competency and as relevant to the role
• Assist with fluid and nutritional/dietary needs of patients and accurately record nutritional and fluid intake including completion of menu cards and recording intake and output of fluids and solids.
