About the Role: As an ACP, you will take the lead in assessing, diagnosing, and managing care for patients registered with our practices—working both autonomously and as part of a wider clinical team. You’ll help shape and modernise care pathways, ensuring safe, effective, and patient-centred services across traditional professional boundaries.
You'll play a key leadership role within the practice, sharing responsibility for clinical governance alongside the Lead Nurse and helping inspire a high standard of care throughout the team.
Your Day-to-Day Responsibilities:
- Manage full episodes of patient care, including minor illness and urgent assessments
- Make autonomous clinical decisions in diagnosis and treatment
- Collaborate within a multidisciplinary team including GPs, nurses, PAs, wellbeing advisors, and health coaches
- Lead by example in clinical quality, patient care, and safety
- Support clinical governance processes and service improvement initiatives
- Utilise EMIS for accurate documentation and case management
What We’re Looking For:
- Current NMC / HCPC / GPhC registration
- MSc in Advanced Clinical Practice (or equivalent)
- Independent Prescriber qualification
- Proven experience in primary care, including minor illness management
- Strong clinical reasoning and autonomous decision-making abilities
- A flexible, team-oriented mindset and willingness to adapt to new models of care
- Excellent communication and leadership skills
