Crynodeb
This post is a rotational post between the community falls team and the bedded rehabilitation team in Inner North West London Division of CLCH and we have newly started an agreement with Imperial NHS Trust where this post will also rotate to Imperial. The rotations are 6 months in length. To provide occupational therapy, evidencedbased assessments and treatment for referred clients, in a variety of settings including acute hospital, clinics, clients own home, residential care homes, community centres and rehabilitation units as part of a multi-disciplinary team, planning, implementing, and evaluating interventions for groups or individuals in conjunction with senior occupational/ physio therapists. This will include providing visible and professional support to ensure the delivery of high-quality care and rehabilitation.
Tobe responsible forthe planning and delivery of occupational therapy and multidisciplinary assessments and interventions to patients within thefallsprevention/ rehabilitation services prioritising clinical needs to provide an effective service.
To undertake occupational therapy assessments, for a designated caseload, addressing occupational performance, impairmentsidentifiedand enabling the client in areas of self-care, productivity, and leisure.
To work with a caseload of patients of varying complexities and needs
To independently carry out rehabilitation programmes and interventions aimed at improving ormaintainingindependence.To formulate intervention plans which are aimed at maximising independence, safety, reducing the falls risks and reflect the clients' own goals.
To plan and implement individual and/or group interventions in collaboration with the client using graded activity to achieve therapeutic goals.
To undertake assessments of clients' home environments,identifyingany modifications to the environment or assistive technology to support independence and safety.
To undertake non-complex moving and handling assessments, multifactorial falls risk assessments prescribing equipment where necessary and handing over to clients and theircarersin the safe use of the equipment.
