Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated.

Hours: 30 hours per week
Salary: £25,027.20 per annum (Pro Rata) £12.80 per hour

Based at: Haverfordwest, Pembrokeshire

Role Purpose:

Pembrokeshire Housing services have been developed in partnership with Pembrokeshire County Council, Hywel Dda and Pembrokeshire Housing Support Grant Team.
Adferiad’s Support Accommodation project staff support individuals with a serious mental illness. The housing services are temporary supported move on accommodation projects and the support is delivered either in shared accommodation, supported housing setting in Haverfordwest or in people’s own homes in the locality. Pembrokeshire Supported Accommodation projects include a 24 hour shared supported accommodation to six occupants who are diagnosed as experiencing a serious mental illness and have been assessed as needing high levels of support to help with their recovery. Adferiad provides individuals support in a second shared house where people receive a lower level of housing related support and thirdly, Adferiad support clients who live in the five additional one bedroom flats in the locality.
The aim of the Adferiad staff team is to encourage and enable clients to reach a greater level of independence over a reasonable period of time, and then to facilitate their move on to more independent living in the community.
The service is staffed 7 days a week, 24 hours per day with on-site waking nights, sleep-in cover and staff are on a shift pattern rota which includes unsocial hours.

Mae gwasanaethau Tai Sir Benfro wedi cael eu datblygu mewn partneriaeth â Chyngor Sir Penfro, Hywel Dda a Thîm Grant Cymorth Tai Sir Benfro.
Mae staff prosiect Llety Cymorth Adferiad yn cefnogi unigolion ag afiechyd meddwl difrifol. Prosiectau llety symud ymlaen dros dro â chymorth yw’r gwasanaethau tai a chaiff y cymorth ei ddarparu naill ai mewn llety a rennir, lleoliad tai â chymorth yn Hwlffordd neu yng nghartrefi pobl yn yr ardal leol. Mae prosiectau Llety â Chymorth Sir Benfro yn cynnwys llety â chymorth a rennir 24 awr y dydd i chwech o breswylwyr y canfuwyd eu bod yn profi salwch meddwl difrifol ac sydd wedi cael eu hasesu fel rhai sydd angen lefelau uchel o gymorth i helpu gyda’u hadferiad. Mae Adferiad yn darparu cymorth i unigolion mewn ail dŷ a rennir lle mae pobl yn cael lefel is o gymorth cysylltiedig â thai ac yn drydydd, mae Adferiad yn cefnogi cleientiaid sy’n byw yn y pum fflat un ystafell wely ychwanegol yn yr ardal.
Nod tîm staff Adferiad yw annog a galluogi cleientiaid i gyrraedd lefel uwch o annibyniaeth dros gyfnod rhesymol o amser, ac yna hwyluso eu symud ymlaen i fyw'n fwy annibynnol yn y gymuned.
Mae'r gwasanaeth yn cael ei staffio 7 diwrnod yr wythnos, 24 awr y dydd gyda nosweithiau effro ar y safle, cyflenwi cysgu i mewn ac mae staff ar rota patrwm shifftiau sy'n cynnwys oriau anghymdeithasol.
