Job Description (27hrs per week over 3 days 9am-5pm):
Care Planning & Service Delivery

Co-ordinate personalised care packages for service users in line with assessed needs and personal outcomes.

Ensure all care plans are reflective of the Health and Social Care Standards and reviewed regularly.

Support the development and implementation of risk assessments and care documentation.

Respond promptly to changes in care needs or emergencies, ensuring appropriate adjustments and communication.

2. Scheduling & Continuity

Manage the electronic rostering of care staff to meet service demands with minimal disruption.

Monitor staff attendance, punctuality, and continuity of care in line with client preferences.

Ensure robust holiday, sickness, and emergency cover plans are in place to prevent service disruption.

3. Staff Support & Communication

Act as the first point of contact for care staff in the field, providing guidance and support.

Liaise with care staff, service users, families, and health professionals to ensure care is coordinated effectively.

Support induction and shadowing for new staff in collaboration with the Registered Manager and Training Lead.

4. Compliance & Quality Assurance

Ensure services are delivered in line with Care Inspectorate Scotland regulations, Adult Support and Protection (Scotland) Act, and company policies.

Participate in audits, spot checks, supervision sessions, and service reviews as required.

Accurately maintain service user records and staff communication logs.

5. Safeguarding & Risk Management

Report any safeguarding concerns immediately, following internal protocols and in compliance with legal requirements.

Support a culture of safety, dignity, and respect through clear communication and reflective practice.

6. On-Call & Emergency Support

Participate in the out-of-hours on-call rota, responding effectively to urgent care needs or staff queries.

Person Specific:
The ideal candidate will have strong IT skills and be proficient in using Google Drive, Microsoft 365, email communication, and basic payroll systems. They should also be based locally within North Lanarkshire or the surrounding area to ensure responsiveness and familiarity with the community.
