The Bromley Older People’s Memory Service delivers services to people across the Bromley borough in line with the National Dementia strategy and MSNAP accreditation standards. We see service users and their carers at Bridgeways and in their homes, delivering a range of interventions via individual and group work.

This post will provide Occupational Therapy input into the Memory Service. The post holderwill be responsible for following the OT care pathway by undertaking effective assessment, goal setting, intervention and discharge planning with service users and their carers. The post holder will work closely with the Band 6 Occupational Therapist who will provide clinical supervision, alongside collaborative working with the multi-disciplinary team (MDT). The post holder will help to co-facilitate groups, with an emphasis on the Cognitive Stimulation Therapy Group. The postholder will need to be self-motivated, organised and able to work autonomously.

• To manage a clinical caseload
• To work as a member of the MDT
• To support the development of evidence based occupational therapy practice e.g. cognitive rehabilitation and utilising occupational therapy outcome measures where appropriate
To provide occupational therapy specific assessment and interventions via individual and group work related to:

-Self care

-Leisure

-Work
• To contribute to service development and any research/audit activities in collaboration with the Band 6 OT
• To carry out home visits
• To work closely with Occupational Therapy students and undertake practice educator responsibilities when required as per the fair share model
• Contribute to and carry out risk assessment and management plans e.g. falls prevention, managing risks associated with daily living and dementia
• To provide Occupational Therapy advice to the MDT
• To work independently without direct supervision

· To manage a caseload of service users with Dementia/Mental health needs

· To work with service users in a variety of settings to provide effective assessment and interventions

· To work with service users to identify Occupational Therapy goals as part of the multi-disciplinary care plan

· To plan and implement service user led individual and/or group interventions where appropriate, using graded activity to achieve therapeutic goals

· To ensure service users health, social, cultural and spiritual needs are considered at all times

· To assess for and provide equipment to enhance independence in daily living skills for service users with a physical disability in addition to their mental health needs. To refer or signpost onwards when necessary to other services e.g. housing associations and social services

· To monitor, evaluate and modify treatment to measure progress and identify outcomes

· To apply a high level of understanding of the effect of disability and provide training and advice on life skills to maximise functional ability

· To assess the occupational needs of service users and establish and evaluate appropriate treatment interventions

· To support the band 6 OT with identifying appropriate & inappropriate referrals and prioritise workload accordingly

· To manage effective discharge ensuring service user and all relevant agencies are given relevant information

· To contribute to and carry out risk assessment & risk management plans

· To work as part of the MDT and attend all relevant clinical meetings
