Bank Community Neurorehabilitation OT's

Spread your wings, Bexley Community Neurorehabilitation Team is looking to appoint a motivated and committed Specialist Occupational Therapists, with a keen interest and proven experience of working with adults with neurological conditions.

Ideally you will be an existing B6 OT wanting to further develop your specialist skills in neurology and community working. However we also welcome applications from experienced Band 5’s for potential consideration.

The team provides a service in clients’ own homes, care homes and the local community and provides a great opportunity for joint working, joint goal setting and regular in-service training. In exchange for your professional skills, passion and commitment to patient centred care, we offer you a supportive, friendly team where you will be listened to, feel valued and be developed as a specialist clinician. With support and training from the 8a Clinical Specialist and Highly Specialist Band 7 OT’s, you will provide specialist neurological OT assessment and treatment to clients presenting with highly complex neurological conditions including Stroke, MND, MS, Head Injury, and Parkinson’s. It is essential that you hold a full UK driving license and have access to a suitable vehicle for business purposes. Mileage is paid at 45p per mile.

This is a Bank role and work is on an as and when required basis

You will be part of a busy community multidisciplinary Neuro-Rehabilitation team in the London Borough of Bexley, working from home and flexible working is supported by the trust.

The service encourages continued professional development, with in-service training within the team and you will have the opportunity to work closely with other colleagues in the South East London sector.

You need to be a confident, autonomous practitioner with high level communication skills, a flexible problem-solving approach, good clinical reasoning skills, and be efficient in your time and people management

Main duties

To provide specialist occupational therapy input to clients as part of the specialist Community Neurorehabilitation service.

To work with specialist nurses and other health and social care practitioners to manage those with neurology long term conditions.

To supervise Band 5 occupational therapists/ therapy assistants and occupational therapy students.

To be responsible and accountable for assessment, diagnosis and treatment for a wide variety of conditions including complex cases, in the community as an autonomous and lone practitioner to clients.

To be able to demonstrate specialist knowledge levels and skills in neurology conditions and interventions.

To demonstrate theoretical knowledge of multiple pathologies and impairments in the assessment of community clients, often in the presence of incomplete or conflicting referral information

To undertake effective support, guidance, supervision, and appraisal of junior staff

To identify possible areas of development for the Occupational Therapy service in the service and communicate this to Lead therapist

To provide training and education programmes for allied health care professionals, rehabilitation assistants, and social care assistants, other care trust staff in the form of tutorials and presentations.

To maintain continuing professional development by participating in regular in-service training, supervision, reflective practice and training, giving feedback to the team where appropriate to disseminate learning. To utilise communication and teaching skills to educate the diverse and multicultural population in a range of disease specific therapeutic techniques, principals and guidelines.

To be responsible and accountable for assessment, diagnosis and treatment for a wide variety of conditions including complex cases, in the community as an autonomous and lone practitioner to clients.

To be able to demonstrate specialist knowledge levels and skills in neurology conditions and interventions.

To demonstrate theoretical knowledge of multiple pathologies and impairments in the assessment of community clients, often in the presence of incomplete or conflicting referral information to form accurate analyses and diagnoses Utilize effective clinical reasoning skills and evidence-based practice to develop individual specialist rehabilitation management care plans.

To assess for and provide equipment to enhance independence in daily living skills for service users To monitor, review and continually re-assess clients complex care plans and objectives against outcomes measures to facilitate modification of packages of care/targets as appropriate to enable achievement of their goals.

To carry out clinical risk assessment and complete risk management plans relevant to the clinical group To seek guidance from more experienced Physiotherapist/ Occupational Therapy colleagues regarding the treatment and management of clients with highly complex presentations.

To work in a co-ordinated, manner, communicating effectively with other team members and the wider clinical network involved in client care.

To provide advice and education to client, carers and relatives based on clinical evidence. To attend and contribute to meetings/ case conferences relating to client best care and wellbeing

To prioritise caseload against team guidelines and standards. Work with specialist neurology nurses and Neurorehabilitation Manager in developing the services for those with neurology long term conditions.

To identify and manage where appropriate the mental / emotion health of the client, with guidance from more senior staff where needed.

To maintain up-to-date clinical written records using trust systems and computerised database in accordance with local and professional standards; and also writing comprehensive discharge and progress reports to referrers.

To manage a clinical caseload and work as a member of the MDT To support the development of evidence-based OT practice To contribute to service development Contribute to and carry out risk assessment & risk management plans To provide specialist OT advice to the MDT To maintain competency in manually handling people and therapeutic manual handling To ensure that quality standards relating to occupational therapy are continually improved.

To identify possible areas of development for the occupational therapy service in the Specialist Community Neurorehabilitation team, and work with senior therapists and team lead in developing them.

To keep abreast of evidence based and best practice To ensure good working knowledge of national and local standards, and monitor quality through participating in clinical audit and evaluation work as required Management responsibilities

To flexibly plan and organise own time and that of therapy assistants. To achieve the effective daily management of a caseload of clients, including prioritising clinical work and balancing other client related and professional duties

To be flexible in the undertaking of any other duties as requested by line managers to meet the changing needs of the service

To attend and participate in Specialist Community Neurorehabilitation team or Trust meetings, and case conferences as required

To comply with professional guidelines, codes of conduct of practice, Trust, and departmental policies

To exercise good personal time management, punctuality, and consistent reliable attendance
