About this role…
We are looking for a Support Worker to join the East Surrey Outreach Service.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
The Thames Reach Outreach teams operate to help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
.Complete early and late shifts, responding quickly to referrals for people experiencing rough sleeping across four different districts.
.Work closely with Lead Workers to identify the fastest and most suitable routes off the street for individuals and then create plans to help them achieve this.
.Offer direct assistance to people sleeping rough, helping them transition away from the streets and into suitable accommodation.
.Work alongside other organizations to ensure clients receive appropriate support to access and maintain accommodation.
.Attend and represent the East Surrey Outreach Service team at various multi-agency meetings.
.Be responsible for accurately documenting all completed work within specified timeframes. Be a proficient user of Microsoft Office applications, including databases.
You will have:
.A strong dedication to delivering a high standard of service is essential.
.Experience working with people who have experienced rough sleeping or other vulnerable adults, and a track record of supporting them to access services.
.Excellent organization and time management to remain outcome-focused, manage a high number of cases, and complete tasks promptly and efficiently.
.Good communication skills and confidence in public speaking, particularly when representing Thames Reach externally.
.Confidence in building strong working relationships with partner agencies, along with the ability to challenge them when necessary, is crucial.
.Strong IT skills, including experience in using Outlook, online recording systems, Excel, and Word, and be comfortable using a tablet and smartphone for work.
