Crynodeb
Main Responsibilities: Provide advanced clinical assessments, diagnoses, and treatment plans for patients presenting with acute conditions. Undertake autonomous clinical consultations, including prescribing following policy, patient group directives and local pathways and referring as appropriate. Support the management of patients with chronic diseases in line with QOF and local guidelines. Contribute to clinical audits, quality improvement projects, and multidisciplinary team meetings. Work collaboratively with the wider practice and PCN team including GPs, practice nurses, receptionists, pharmacists, and social prescribers. Deliver person-centred care and health promotion, tailored to individual patient needs. Ensure that personal and professional clinical standards are maintained. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care. Working Hours: 20 hours per week, with one early start and one later finish to support Enhanced Access.
