Clinical To be a member of a multi-disciplinary team, contributing in all its forums, attending meetings and case conferences for patients. To be responsible for two teams of staff in order to ensure standards are maintained. To function as a member of a multi-disciplinary team taking appropriate responsibility for the scrutiny of diligence. To link closely with Care Co-ordinator in the delivery of care under Care Programme Approach. To promote a collaborative approach to patient care within the team providing support to junior staff. To assist the Modern Matron, Ward Managers and ward staff in the introduction of an effective care delivery system devised to meet the needs of patients and the wards. To act as clinical role model, demonstrating high standards of nursing practice. To ensure close links with hospitals that are out of area to identify/resolve clinical issues. To participate in clinical supervision as supervisor. To maintain good working relationships with other agencies. To maintain confidentiality in respect of the patient / client at all times. Communicate clearly with patients, their families and all other mental health professional groups. To work with the standard of the triangle of care Actively participate in clinical governance and quality improvement initiatives, eg. clinical audit, clinical policy development and Quality Improvement Play an active role in providing cross cover between the Referral Coordination and the Place of Safety Team. Reporting on the nursing assessment of potential admissions to both wards in a timely manner. Provide, receive and analysis complex information from a range of sources Assess and make decisions in relation to patient risk. Key Tasks and responsibilities of the Post Managerial To ensure that safeguarding policies are followed. To ensure effective leadership and supervision of the Referral Coordinators and Place of Safety Team. To participate in the Individual Performance Review as Line Manager of the teams. To participate in the Senior Nurse meetings on a regular basis. To uphold Nursing and the Trusts Operational Policies and Standards. To be aware if the Health and Safety at Work Act and the implications of the work environment. Personal and Professional Development To participate in Individual Performance Review. To received supervision regularly for clinical work. In consultation with the Modern Matron to attend meetings, conferences, study days as appropriate to need. To keep up to date with issues and developments in nursing and the wider NHS. To be responsible for your own training and clinical supervision needs. Key Tasks and responsibilities of the Post Liaison To provide relevant and timely information to colleagues and other agencies. To liaise effectively with the wards and departments in Glenbourne. To maintain effective links with Community Nurse Teams and all other agencies. To maintain effective communication with the Acute Trust (Derriford Hospital) and give advice/practical help in difficult situations. Key Tasks and responsibilities of the Post Administration To maintain up-to-date clinical records. To provide reports regarding performance measures eg POS for the police and CCG. To maintain accurate and prompt recording of accident and incident forms. Ensure incident forms for your department are investigated and closed in a timely manner. To maintain the legal requirements of the Mental Health Act 1983. To have a good working knowledge regarding DOLs, capacity assessment and understanding of executive capacity To ensure effective use of the electronic computer based system, for example Systm 1 and Excel. To audit documentation and provide monthly reports to the Modern Matron using Excel. To ensure that key local and national policies are implemented at a local level giving support where necessary. Notes This Job Description is not exhaustive and may change as the post develops, but such change will not take place without consultation between the postholder and his / her manager. This job will be reviewed within the IPR process on an annual basis. The postholder is required to conform to Livewell Southwest policies on Health and Safety, Fire procedures and confidentiality and to attend any training sessions as required. The Livewell Southwest has adopted no smoking and no alcohol policies for staff, which applies to all posts. Details of the policy are available on request and will be included in the statement of main terms and conditions of staff appointed. The postholder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/Chief Executive Office).
