Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Anaesthetics, Surgery, Critical Care and Renal (ASCR) Division to achieve a high standard of efficiency and effectiveness Responsible for the provision, management and development of a highly specialist pharmaceutical service to the Weight Management Team Supporting and deputising for the pharmacist lead for medical weight management Managing the pharmaceutical care of bariatric surgery patients in the pre and peri-operative period, in conjunction with the lead Band 8a Weight Management & Bariatric Surgery Pharmacist Independently assess and prescribe drugs to treat obesityin a clinic setting Plan peri-operative medication management in conjunction with the patient and the wider multidisciplinary team Prepare prescribing information and documents prior to admission and review based on joint decision making with the patient on the day of surgery Clinically screen prescriptions to ensure the safe, efficient, evidence-based and rational use of medicines in line with the BNSSG Formulary and departmental policies so that risks to patients are minimised Contribute to the overall development of the clinical pharmacy service through active participation in relevant meetings. Work closely with all members of the pharmacy department and other healthcare professionals. Attend consultant and ward board rounds whenever possible to provide pharmaceutical expertise Demonstrate strong leadership skills across all elements of the role Provide reports on the use and cost of medicines to the surgical directorate as appropriate and analyse these to advise on strategies to ensure cost-effective use of resources. Write relevant protocols, procedures and guidelines for use within the Weight Management Clinics and Pre- and Post-Bariatric Surgery and surgical specialist environments Provide pharmaceutical expertise to any multidisciplinary working groups considering protocols, policies, guidelines and integrated care pathways in the specialist area Assimilate and analyse clinical trial information on new pharmaceutical products intended for use within the speciality for submission to the Area Prescribing and Medicines Optimisation Committee (APMOC). Ensure that an accurate medication history is obtained for each patient using the appropriate sources (GP, community pharmacist, patient/carer, clinic notes, nursing home, electronic patient record systems) Assist with prescribing for weight loss clinic patients and inpatients to enable appropriate patient flow Deals with high level pharmaceutical queries and provide advice and information to patients/carers/nursing and medical staff in connection with drug treatment Provide a comprehensive clinical ward pharmacy service to the speciality and work with stakeholders to optimise resources and improve patient flow Work closely with internal and external stakeholders, where necessary to ensure timely and seamless discharge of patients Document all major interventions made Investigate and support the recording of adverse drug reactions and support the documentation of patient allergy in appropriate clinical records Audit and risk manage selected areas of practice to promote continuous quality improvement by affecting change in practice. Participate in ASCR Divisional and Weight Management & Bariatric Surgery clinic audit meetings Maintain and develop medicines management systems in conjunction with patients/carers, ward staff and pharmacy staff. Supervise and contribute to the development of medicines management technicians and/or other dedicated pharmacy staff Contribute to the training of pharmacy undergraduates, medical students, junior doctors, pre- registration pharmacists, pharmacy technicians, allied health professionals and medical and nursing staff as appropriate. Train and mentor clinical pharmacy diploma students on aspects of the specialist area Act as an appraiser in the Pharmacy appraisals programme Ensure that all relevant procedures are adhered to, reviewed regularly and that all necessary records are completed Undertake to follow all departmental procedures in accordance with current local practices and legislative requirements Provide professional cover in the dispensary and surgical inpatients on a sessional basis Keep up to date with developments in Pharmacy to maintain a high level of professional knowledge and competence. Attend study days and courses as necessary for the development of the service and the individual Maintain a record of CPD Work with the specialty to comply with national and local patient safety drug alerts and initiatives and help with the implementation and recommendation of associated action plans Provide comprehensive pharmaceutical care to bariatric surgery and weight management patients in all clinical areas Undertake any other duties as required by the Director of Pharmacy or their deputy
